Requirements
  • A Doctor of Pharmacy degree or RPh is required.
  • Knowledge of pharmacokinetic principles for medication dosing is required.
  • Monitor patients for response to medications.
  • Identify and document sentinel or serious events, near misses, medication errors, and adverse drug reactions in the electronic reporting system.
  • Promote and assure compliance with the hospital formulary and policies and procedures defined by the P&T Committee.
  • Provide care appropriate to the patient's chronological age, developmental age, social maturity, and cultural background.
  • Registered Pharmacist license in Pennsylvania is required.
  • Certification is required based on primary work location.
Responsibilities
  • Oversee the medication distribution system for the assigned area and identify, prevent, and resolve drug therapy problems by ensuring medications are indicated, effective, safe, and convenient for patients.
  • Interact with co-workers, visitors, and other staff consistent with Jefferson's values.
  • Assure the appropriateness of drug therapy and compliance with hospital policies.
  • Oversee the distribution of medications throughout TJUH.
  • Support implementation of processes to assure the quality of Pharmacy services.
  • Train staff as assigned, including residents, interns, technicians, and new pharmacists.
  • Precept student pharmacists as assigned, in accordance with the Preceptor Development & Evaluation Program policy.
Desired Qualifications
  • One year of PGY1 Pharmacy Residency is preferred.
  • Microsoft Word and Microsoft Outlook skills are preferred.
