Gentiva

In-home hospice, palliative, and health services

Gentiva provides in-home hospice, palliative, and home health care through a nationwide network of locations. It delivers care by coordinating in-home visits from clinicians and therapists, including skilled nursing, therapy services, and pain and symptom management, with an Advanced Illness Management model for patients not yet eligible for hospice. Gentiva differentiates itself with an integrated in-home care platform and strategic growth through acquisitions, such as ProMedica’s home health, palliative, and hospice operations. Its goal is to help patients stay at home while receiving comprehensive care and to reduce avoidable hospitalizations for families and caregivers.
