Requirements
  • A Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential from the Commission on Dietetic Registration in good standing is required to provide and bill for medical nutrition therapy under Medicare.
  • Active state licensure or certification is required where applicable.
  • Experience caring for older adults is required.
  • Ability to practice via telehealth is required.
Responsibilities
  • Assess older adults' nutritional needs and build personalized, evidence-based nutrition care plans.
  • Deliver medical nutrition therapy and coaching grounded in lifestyle medicine, using a whole-food, predominantly plant-based emphasis adapted to each patient's health, culture, and preferences.
  • Support patients managing chronic conditions where nutrition is central, including diabetes, cardiovascular and renal health, and weight and metabolic health.
  • Anchor care plans in the four M's of Age-Friendly care: What Matters, Medication, Mentation, and Mobility.
  • Collaborate with lifestyle and longevity nurse practitioners, behavioral health, and primary care coordination to support the whole person.
  • Help shape the clinical model, including protocols, intake, outcome measures, and the patient experience.

It uses a HIPAA-compliant teletherapy platform to connect Medicare beneficiaries with licensed geriatric-focused therapists for individual and group sessions.
