Requirements
  • Be a graduate of an accredited four-year college with a major in social work, psychology, sociology, or a closely related field; alternatively, have a degree in any field and meet one of the stated experience or coursework qualifications.
  • Alternatively, have one year of full-time experience as a social worker, case manager, or care coordinator in a public or private social service agency.
  • Alternatively, have one year of experience in the education or treatment of people served with developmental disabilities or related conditions.
  • Alternatively, have completed at least one course focused on developmental disabilities and provide an official college or university transcript verifying completion.
  • Have a valid driver's license and an acceptable driving record under company policy.
  • Have a vehicle with current insurance coverage.
  • Successfully clear local and/or stated background checks.
  • Be willing to commute to areas of Hennepin and Scott Counties for potential meetings.
Responsibilities
  • Drive to attend client meetings in Scott County, averaging four client meetings per week.
  • Assess areas of an individual's life where assistance is needed.
  • Offer and coordinate resources for individuals being supported.
  • Monitor service delivery and work with providers to ensure individuals' needs are met.
  • Develop service plans.
  • Attend regular in-person meetings with individuals served and their interdisciplinary teams.
  • Document client-related work in case notes to create billable units.
Desired Qualifications
  • Have one year of full-time experience in the education or treatment of people with developmental disabilities or related conditions, or have completed at least one course specifically focused on developmental disabilities.
  • Possess a Licensed Social Worker (LSW) license or a higher form of Board of Social Work licensure.
