Requirements
- Two years of acute care clinical experience, one year of recent case management experience in an acute or post-acute setting, or a Master's in Nursing with a Case Management emphasis.
- Current licensure with the California Board of Registered Nursing.
- Current CPR recognition.
- Knowledge of assessment, planning, intervention, and evaluation of patient care.
- Completion of staff nurse OCAT and passing the medication administration examination.
- Annual completion of the Skills Fair.
- Understanding of third-party reimbursement methodologies.
- Critical thinking and analytical skills.
- Ability to maintain strict confidentiality of patient information.
- Ability to build positive working relationships with physicians and external agencies.
- Ability to communicate with multiple levels of the organization, including managers, physicians, clinical staff, and support staff.
- Ability to use Microsoft Word, Excel, Outlook, Midas, Chart Max, and Epic computer systems.
- Organizational and multitasking skills.
- Ability to fulfill the essential functions of the position.
Responsibilities
- Coordinate care and services for assigned patients throughout an episode of illness or treatment in collaboration with patients, families, physicians, the patient care team, and payors.
- Use advanced clinical skills to assess, plan, intervene, and evaluate patient care.
- Promote and evaluate effective resource utilization using current clinical knowledge and awareness of community services.
- Assume a leadership role with the patient care team to achieve optimal clinical and resource outcomes.
- Evaluate quality and utilization cases, admission and discharge criteria, observation and one-day stays, readmissions within 30 days, Medicare criteria, and diagnosis-related criteria.
- Participate in ongoing outlier review.
- Coordinate emergency department episodes of care when assigned.
- Ensure patient progress through emergency episodes of care in an efficient and cost-effective manner for the assigned caseload.
- Advocate for patients and families throughout the entire episode of care.
- Maintain availability to patients and families as a resource to facilitate communication among providers and monitor services rendered.
- Remain involved until the patient is discharged, appropriately transferred to an acute care setting, or transferred to an alternate level of care.
- Initiate appropriate clinical pathways based on the admission diagnosis.
- Serve as a resource person for utilization management purposes.
Desired Qualifications
- Bachelor's degree in Nursing.
- Experience in acute clinical case management and utilization review.
- Working knowledge of managed care requirements and hospital information systems.
- Experience in the assigned area.
- Ability to maintain confidentiality of patient information, advocate for patients, collaborate, and delegate when appropriate.
- Initiative to assume fiscal responsibilities.
- Excellent communication and interpersonal skills to establish rapport and professional interaction.
