Requirements
  • The candidate must be at least 18 years of age.
  • The candidate must hold or obtain and maintain a valid driver's license with no more than five points.
  • The candidate must meet the essential physical demands of the position, including frequent walking, sitting, standing, bending, twisting, stooping, kneeling, crouching, pushing, pulling, and reaching; using the hands to handle, finger, or feel objects, tools, or controls; assisting with lifting or moving individuals of any weight with mechanical lifts or other equipment; pushing individuals in wheelchairs; lifting or moving up to 25 pounds occasionally; and implementing emergency interventions as required.
  • The candidate must have effective written and oral communication skills, including the ability to document services provided for billing and tracking purposes.
  • The candidate must be willing to assist with a variety of supports and activities of daily living, including regular exposure to and handling of bodily fluids.
  • The candidate must have basic computer skills, including email and use of Workday and related systems such as Digital Agency.
  • The candidate must hold or obtain and maintain certifications and trainings required by the program assignment and supported individuals, including Non-Crisis Invention training, American Heart Association CPR/First Aid certification, New Jersey Medication Administration certification, and Positive Behavior Supports training within 120 days of appointment.
  • The candidate must be able to develop and maintain professional relationships aligned with the organization's mission and vision.
  • The candidate must remain calm in stressful situations and work in a high-energy, fast-paced environment.
  • The candidate must be able to work beyond regularly scheduled hours or shifts and report as scheduled on holidays, during inclement weather, or during emergencies when required to maintain minimum program coverage.
Responsibilities
  • Assist individuals in maximizing their strengths, interests, and abilities while upholding their rights, providing emotional support, and building positive relationships.
  • Support individuals in meeting personal quality-of-life goals, using person-first communication, encouraging socialization, and facilitating activities that build community connections.
  • Provide continuous supervision, protection, and care, maintain program safety guidelines, facilitate fire-safety learning, and identify and escalate signs of potential harm to self or others.
  • Provide health, wellness, and safety support, including supervision and crisis management in accordance with applicable policies, procedures, and regulations.
  • Provide support in varied settings according to individualized service and behavior support plans, using support skills for people with varying levels of need and applying New Jersey Division of Developmental Disabilities standards.
  • Implement person-centered individualized plans, including Behavior Support Plans, Individualized Service Plans, Staff Action Plans, and Life Plans, according to required supervision levels and safeguards.
  • Maintain a clean, safe, and organized therapeutic environment.
  • Use safe handling procedures and implement infection-control protocols.
  • Provide personal care and assistance with activities of daily living, including bathing, dressing, toileting, laundry, and housekeeping.
  • Teach and develop habilitative and rehabilitative skills.
  • Support person-centered advocacy, communication, self-expression, dispute resolution, and ethical decision-making.
  • Supervise or plan and prepare balanced meals and snacks in accordance with individual dietary restrictions and guidelines, including modified diets and liquid consistencies.
  • Transport and escort individuals to recreational activities, medical appointments, home visits, volunteer or employment sites, and other community or therapeutic activities; help establish schedules and opportunities.
  • Assist individuals with purchases and safeguard personal allowance and other funds.
  • Administer medication as prescribed and in accordance with applicable guidelines.
  • Assist with medication inventory, cabinet checks, over-the-counter inventory, and timely reordering of medication and supplies.
  • Assist with scheduling medical appointments and ensure follow-up appointments are scheduled and attended on time.
  • Use petty cash and house funds, including store cards, and maintain required purchase documentation and receipts.
  • Operate agency vehicles to transport individuals and update vehicle logs and maintenance sheets while coordinating appointments with the supervisor.
  • Develop specialized clinical knowledge for individual needs, including support for adults with challenging behaviors, autism spectrum disorders, and complex medical needs.
  • Implement Behavior Support Plans, behavior guidelines, and protocols specified in applicable plans.
  • Collect, record, and summarize data on observable behavior.
  • Observe and report behavioral changes and other signs or symptoms to the clinical or leadership team.
  • Communicate by phone, email, or in person with families, external providers, and other members of individuals' circles of support to provide updates or obtain information.
  • Participate as a member of the support team by providing feedback, communicating program needs, and addressing conflicts constructively.
  • Complete and maintain required trainings and certifications and apply the acquired skills.
  • Review communication logs daily and record events, progress notes, services, and other information using prescribed communication systems; collect and enter operational data in an electronic health record or medication record.
  • Complete required paperwork and documentation, including notifications, logs, incident reports, file updates, maintenance requests, phone calls, and incident tracking.
  • Use foundational teaching skills such as Explain, Model, Observe and Evaluate, task analysis, essential skills, supports, modifications, and progressive teaching.
  • Understand behavior as communication and use person-centered approaches to support positive interactions and prevent escalation.
  • Report maintenance, upkeep, and safety concerns at facilities to the Program Supervisor or designee.
  • Consult with program leadership, clinical staff, and direct support staff to coordinate care and participate actively in team meetings.
  • Use approved organizational and New Jersey Division of Developmental Disabilities verbal, environmental, and physical intervention techniques, including takedowns, to respond to challenging or self-injurious behavior.
  • Provide coverage across assigned regions and service settings, including off-site locations such as hospitals, and maintain minimum staffing coverage.
  • Participate in treatment-plan development when assigned.
  • Comply with federal, state, local, and other regulatory requirements, including the Health Insurance Portability and Accountability Act.
  • Perform other assigned duties.
