- Develop and expand referral relationships with hospitals, skilled nursing facilities, physicians, and discharge planners aligned to revenue targets.
- Ensure delivery of quality patient and family services, including clinical oversight in home, hospital, and long-term care settings.
- Supervise executive directors, medical staff, volunteer programs, and interdepartmental teams to optimize care coordination and performance.
- Serve as a visible ambassador for the organization in the community, manage public relations initiatives, and resolve public or patient concerns.
- Participate in ongoing quality assessment and improvement programs to drive high-quality hospice care.
- A registered nurse degree.
- A bachelor's degree in Healthcare Administration, Business, or a related field.
- Multi-site operational leadership experience.
- Three or more years of direct healthcare services leadership experience.
- Experience with HomeCare HomeBase.
- Strong communication and organizational skills.
- Critical thinking and problem-solving ability.
- Experience providing patient-centered care and working with interdisciplinary teams.
- Ability to balance strategy, operations, and field leadership.
