Requirements
  • A bachelor's degree in social services or a related field.
  • Two years of experience in case management, job development, or crisis intervention with youth, plus experience in community building, event coordination, and public speaking.
  • Maintain required certificates, including First Aid and CPR.
  • Hold a valid Texas driver's license with a clear driving record and reliable transportation.
  • Pass pre-employment and random drug screens.
  • Provide proof of work eligibility status upon request.
  • Pass pre-employment and biennial criminal background and motor vehicle checks; fingerprints are required if the applicant has lived out of state within the past five years.
  • Communicate effectively in writing and verbally in English.
  • Maintain the computer literacy required for the position.
  • Work effectively and without intensive supervision, independently and as a member of a multidisciplinary team.
  • Respond sensitively and competently to the service population's cultural and socioeconomic characteristics.
  • Travel as needed.
Responsibilities
  • Provide case management to eligible young adults ages 16 to 21.
  • Develop Individual Service Plans, meet with clients, assess progress, and maintain current client files.
  • Obtain DFPS youth participation approval through authorization form 2054 from DFPS PAL staff before providing services.
  • Follow DFPS guidelines and policies concerning PAL Transitional and Financial Services.
  • Maintain an organizational management structure that promotes best practices and optimal service delivery.
  • Monitor and maintain an internal policy and procedure manual based on federal requirements, grant procedures and requirements, and agency policies and procedures.
  • Guide, lead, develop, and mentor direct-reporting staff through regular communication and evaluations.
  • Ensure supervised-program staff receive the training necessary to provide high-quality programming.
  • Work effectively with the Director of Community Based Services.
  • Attend required client staffing, department, and agency meetings.
  • Provide crisis management.
  • Make referrals to outside agencies as necessary and provide follow-up.
  • Present at conferences and other professional meetings on transition services.
  • Maintain confidentiality with clients and staff.
  • Complete initial intake assessments for youth seeking PAL Transitional and Financial Services.
  • Help clients develop and sign their Individual Transition Plan, or sign a plan review, during each face-to-face contact.
  • Maintain client files according to BCFS and DFPS standards.
  • Enter required Salesforce data within two business days of client contact and submit monthly reports as specified by the Program Director.
  • Track and achieve program outcomes.
  • Submit monthly and quarterly reports to the Director of Community Based Services.
  • Conduct monthly case reviews to ensure programs follow guidelines and contact requirements.
  • Submit receipts, case notes, expense reports, mileage reports, and other required documentation on time for budgeting and billing.
  • Perform other job duties as assigned according to grant requirements.
