Requirements
  • A bachelor's degree in social work, psychology, education, rehabilitation, recreation therapy, sociology, nursing, or a related field approved by the Illinois Department of Human Services; alternatively, qualifying pre-2009 or temporary candidates may have five or more years of developmental disabilities experience and a high school diploma.
  • At least one year of experience working with people with developmental disabilities.
  • At least one year of supervisory experience.
  • Successfully complete company-provided CPR, first aid, Crisis Prevention Institute, direct support professional, and medication administration training within the first four months, with annual renewals where required.
  • Obtain an Illinois Food Service and Sanitation certificate within the first six months and renew it as required.
  • Pass the state-required TABE literacy assessment before employment.
  • Be proficient with Microsoft Word, Excel, and Outlook.
  • Pass criminal background, state and federal registry, DCFS Abuse and Neglect Tracking System, and driving history checks.
  • Maintain a valid driver's license and meet the applicable age and company-policy criteria to drive company vans.
  • Use a personally insured vehicle for company-related travel.
  • Employees without a DHS-approved degree and experience must have another qualified QIDP staff member review, approve, and sign treatment plans, MPNs, and staffings, and cannot conduct DHS training.
Responsibilities
  • Manage residential housing programs that support people with disabilities in achieving independence in a home-based environment.
  • Assist residents with daily living activities, transportation, medication documentation, hygiene, shopping, cooking, cleaning, and laundry.
  • Plan, coordinate, supervise, evaluate, and document resident treatment services and care.
  • Ensure regulatory compliance, funding-agency agreement compliance, accurate documentation, safety, house rules, inspections, and facility policies and procedures.
  • Write and update IHP, PP, ISP, and IATP documentation and chair the IDT.
  • Supervise residents’ finances and bank accounts as prescribed by the IDT.
  • Participate in prospective resident screening, interviews, and admission decisions.
  • Develop and integrate treatment plans according to Active Treatment guidelines using facility, community, and day-program resources.
  • Support family and guardian participation and visitation, and provide emergency aid or arrange medical and dental evaluations and treatment as required.
  • Provide a clean, safe, pleasant, and homelike living environment; record accidents, injuries, and illnesses; and participate in the Human Rights Committee.
  • Provide personal protective equipment and comply with company safety and Occupational Safety and Health Administration standards.
  • Schedule and supervise routine maintenance and minor repairs and report major repairs or facility needs to administration.
  • Approve purchase orders, supervise inventory controls, submit bills, approve expenditures, and assist with budgets and financial reports.
  • Assist with residential-facility public relations, site visitations, surveys, and capital expenditure requests.
  • Lead and supervise 8-12 staff members, including hiring, training, empowerment, scheduling, communication of expectations, performance monitoring, appraisal, mentoring, and development.
  • Plan, organize, implement, direct, and evaluate assigned site activities and performance and enforce systems, policies, and procedures.
