About the job
- Registered Nurse license and at least 3 years of healthcare experience; hospice or related-setting experience is preferred.
- A bachelor's degree is strongly preferred.
- Leadership or management experience is a major plus.
- A valid driver's license is required.
- Lead and oversee an interdisciplinary team of nurses, aides, social workers, chaplains, and other team members.
- Ensure adherence to hospice regulations and standards.
- Facilitate communication between medical staff and the care team.
- Lead interdisciplinary meetings and document patient needs and care plans.
- Develop and update patient care policies and emergency procedures.
- Manage staff scheduling and performance reviews, and foster a positive work environment.
- Coordinate communication with families, physicians, and other providers.
- Analyze quality metrics and manage a patient caseload as needed.
- Participate in after-hours and on-call duties to ensure continuity of care.
- Maximize resource utilization and promote a collaborative work culture.
