Requirements
- An active, unrestricted New York registered nurse license and New York State Education Department Nurse Practitioner certification in a specialty covering older adults, such as Adult, Gerontology, or Family Health.
- Current national Nurse Practitioner board certification.
- Active Drug Enforcement Administration registration, New York electronic prescribing setup, and completion of New York's mandatory prescriber education of three hours every three years.
- More than 3,600 hours of Nurse Practitioner practice, or the ability to practice under a written collaborative agreement and practice protocols with a physician, as required in New York.
- Clinical experience caring for adults aged 65 and older.
- Experience with Medicare documentation and evaluation and management coding.
- Own malpractice insurance of at least $1 million per occurrence and $3 million aggregate that covers in-person care, or the ability to obtain it before the first site day.
- Comfort presenting to groups and speaking with older adults and their families.
- A valid driver's license and reliable transportation.
- Ability to complete each community's onboarding requirements, which may include a background check, tuberculosis screening, and immunizations.
- Weekday availability for on-site days in the Capital Region, with hours confirmed in advance.
Responsibilities
- Conduct 45- to 60-minute longevity visits based on a comprehensive geriatric assessment, covering the resident's priorities, medication review and deprescribing, cognition including the Mini-Cog, mood, sleep, nutrition, mobility, fall risk, daily function, and advance care planning.
- Evaluate eligible residents for weight management, including glucagon-like peptide-1 therapy when clinically appropriate.
- Identify residents who may benefit from follow-up care, such as a fuller cognitive review, ongoing medical follow-up, psychiatry, or therapy, and coordinate with their primary care provider.
- Document every encounter accurately and completely to Medicare standards in the electronic health record.
- Represent Total Life professionally with residents, families, and community staff.
- Travel to assigned senior living communities in the Capital Region.
Desired Qualifications
- Experience with comprehensive geriatric assessment.
- Familiarity with cognitive screening and staging tools, including Mini-Cog, Montreal Cognitive Assessment, and Functional Assessment Staging Tool; Montreal Cognitive Assessment certification is a plus.
- Experience in senior living, long-term care, skilled nursing facilities, hospice, palliative care, or house-call settings.
- Experience prescribing glucagon-like peptide-1 medications for weight management.
- Telehealth experience.
- Licenses in more than one state.
- Adult-Gerontology Nurse Practitioner certification, or Family Nurse Practitioner certification with a geriatric focus or training.
