About the Company

Hart Medical Equipment

Post-discharge home medical equipment provider

Hart Medical Equipment provides durable medical equipment (DME) and related support services through a joint venture with major health systems, serving Michigan and Ohio with 29 locations and about 67,000 patients monthly. After hospital discharge, it supplies devices such as CPAP/BiPAP machines, oxygen, mobility aids, and diabetes/ostomy/wound care products, available in-store and via online ordering, billable to Medicare and most insurers with a physician’s order. Its TeleHart telehealth platform offers remote education and training, and the company aims to streamline discharge referrals to support clinicians and ensure continuity of care. Its hospital-partnered discharge embedding and Medicare-contracted status help coordinate transitions from hospital to home and aim to improve patient adherence and outcomes.
