Requirements
  • A high school education is required.
  • Excellent computer skills are required.
Responsibilities
  • Accurately enter all appropriate patient information into the electronic medical record.
  • Schedule all physical therapy and skilled nursing admissions and evaluations.
  • Coordinate scheduling of all home health visits.
  • Monitor compliance with therapy functional assessments and supervisory visits.
  • Monitor staff productivity and communicate with clinical managers.
  • Complete any required audits.
  • Triage intake phone calls and referrals.
  • Assist the intake department as needed.
  • Communicate authorization requests to the appropriate personnel.
Desired Qualifications
  • An associate degree is preferred.
  • Two to three years of home health experience is preferred.
