Requirements
  • Maintain an active Registered Nurse license in the state where the work is performed.

Responsibilities
  • Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
  • Develop, implement, and update individualized plans of care in partnership with patients and families.
  • Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
  • Educate patients and caregivers about disease progression, symptom management, medications, and what to expect at end of life.
  • Coordinate care across the caseload and serve as the primary liaison between patients, families, physicians, and the care team.
  • Participate in interdisciplinary group meetings to ensure comprehensive, holistic care.
