Requirements
  • At least one year of registered nursing experience in acute care hospital, oncology, long-term care, hospice, or home health; school internships and clinical hours may be considered in individual cases.
  • Current license to practice nursing in the state of employment.
  • Current CPR certification.
  • Current automobile insurance and a valid driver’s license.
  • Knowledge of terminally ill patients and their families, hospice care, hospice disciplines, community resources, general nursing practice, managed care principles, and regulatory guidelines including Medicare, Medicaid, and ACHC.
  • Ability to perform patient assessments, communicate effectively, document clinical information, operate nursing assessment tools and office equipment, and maintain transportation for work.
Responsibilities
  • Serve as the primary point of contact for hospice patients and families throughout their hospice journey.
  • Deliver and document skilled nursing care according to individualized plans of care in patients’ homes, assisted living facilities, or other care settings.
  • Perform comprehensive initial, ongoing, in-person, and telephonic patient assessments and reassessments.
  • Develop and update individualized plans of care, including visit frequency and risk identification.
  • Deliver skilled nursing interventions and symptom management aligned with goals of care.
  • Coordinate care with physicians, nurse practitioners, licensed practical nurses, certified nursing assistants, social workers, chaplains, and other interdisciplinary team members.
  • Provide emotional, physical, and spiritual support, education, comfort, and bereavement guidance to patients and families.
  • Teach patients, families, and caregivers about disease processes, self-care, end-of-life care, medication administration, nutrition, dietary needs, hospice philosophy and services, symptom and pain management, and end-of-life expectations.
  • Communicate changes in patient condition and signs or symptoms of decline to the appropriate provider.
  • Support continuity of care by recognizing and supporting home health, palliative, and hospice referrals and transitions between levels of care.
  • Maintain timely and accurate clinical records, including screening and assessment findings, physician orders, progress notes, and care plans.
  • Follow infection-control, regulatory, policy, and compliance standards.
  • Participate in interdisciplinary group activities, quality improvement, scheduling initiatives, on-call services, and patient-support activities as applicable.
