Requirements
  • Active, unrestricted Nurse Practitioner or Advanced Practice Registered Nurse license and all applicable prescribing or furnishing authority required for practice in the state or states in which the Nurse Practitioner will provide care.
  • Licensure in at least one of the following states: Alaska, Arizona, California, Louisiana, Michigan, New Mexico, or Nevada.
  • Experience working with Medicaid and Medicare populations.
  • Strong understanding of social determinants of health and their impact on patient outcomes.
  • Comfort with telehealth platforms and building rapport virtually.
  • Ability to engage patients from diverse backgrounds.
  • Familiarity with state, federal, and payer requirements applicable to Community Health Worker services and referrals, or willingness to learn.
Responsibilities
  • Conduct telehealth clinical evaluations to assess acute and chronic health needs, preventive care needs, and social drivers of health.
  • Determine clinical appropriateness for Community Health Worker services and make referrals for care coordination and community-based support when indicated.
  • Develop individualized care plans addressing clinical needs and social determinants of health factors.
  • Collaborate with Community Health Workers to support patient engagement and follow-through.
  • Address appropriate acute, chronic, preventive, and medication-management needs within the scope of the program, including prescribing and renewing non-controlled medications when clinically indicated.
  • Document visits accurately and timely in accordance with clinical, state, federal, and program requirements.
  • Help patients navigate complex healthcare systems and connect to appropriate medical and community resources.
Desired Qualifications
  • Licensure in multiple states, with willingness to obtain additional state licenses as needed.
  • Experience in primary care, family medicine, internal medicine, urgent care, or chronic disease management.
  • Telehealth experience.
  • Spanish or other language fluency.

Health Net has awarded a $900,000 grant to Pear Suite to help California community-based organisations strengthen their technology infrastructure for delivering community health worker and doula services. The funding will support organisations becoming enrolled Medi-Cal providers and streamline service documentation, referrals, and reimbursement processes. The grant builds on Health Net's partnership with Pear Suite, which has totalled more than $1.9 million since 2023. Pear Suite's platform provides a solution for assessing social needs, coordinating referrals, documenting services, and managing billing for Medi-Cal reimbursement. The initiative aims to enable community health workers and doulas to spend more time directly helping Medi-Cal members with housing support, transportation, and other social needs, whilst reducing administrative burdens. Health Net serves more than three million members across California through its network of over 117,000 providers.
