Requirements
  • Current license to practice nursing in the state of Kentucky.
  • Basic Life Support certification is required.
  • Ability to perform nursing duties involving frequent standing, walking, sitting, hand use, and occasional lifting or moving of up to 50 pounds.
  • Ability to communicate effectively with vulnerable patient populations and maintain patient confidentiality.
Responsibilities
  • Provide comprehensive nursing assessments and document them according to patients' developmental levels and emotional states.
  • Assist providers with obtaining patient histories and anticipating current needs.
  • Plan care by reviewing schedules and charts to prepare for patient encounters.
  • Educate patients, family members, and staff about clinical quality measures and preventive health maintenance recommendations.
  • Perform nursing interventions competently and maintain current nursing skills and professional practice knowledge.
  • Manage medications, including medication reconciliation, refills, and immunization guidelines.
  • Collaborate with the healthcare team to promote population health, health, and wellness.
  • Coordinate care through communication with community organizations, health plans, facilities, and specialists.
  • Participate in Continuous Quality Improvement activities, practice-performance review, quality-outcome improvement, and the organization's advocacy program.
  • Follow organizational administrative, clinical, quality-assurance, and personnel policies.
  • Maintain timely documentation, satisfactory attendance, and communication with the supervisor regarding attendance and assignments.
  • Work cooperatively with providers, administration, and peers while managing multiple tasks.
