- Must be a certified Medication Aide and/or meet the state requirements for medication aide.
- A minimum of one year of experience working with elders or disabled individuals is preferred.
- Must have demonstrated leadership capabilities.
- Knowledge and experience in the assisted living industry and dementia care or skilled nursing is preferred.
- Ability to work weekends, evenings, and flexible hours and be available to customers during peak service delivery days and times.
- Identify ongoing needs and services of residents through the assessment and Personalized Service Plan process.
- Ensure proper follow-through and documentation for residents with a change in clinical care needs.
- Support the Wellness Nurse by completing the clinical sections of monthly wellness visits and Personalized Service Plans to accurately reflect each resident.
- Inform the Wellness Director and/or Executive Director of changes in medication or service level that may result in a higher tier level or potential move-out.
- Assist with obtaining monthly weights and vital signs for each resident before completion of the monthly wellness visit.
- Assist with resident care as needed.
- Monitor the health and well-being of residents and provide leadership support and assistance with resident care and services.
- Assist with medication care and administration, communicate with resident support services and families, and maintain clinical quality assurance while leading, demonstrating, and supporting wellness programs and services.
