Requirements
- The candidate must be a Registered Nurse licensed to practice in the State of New Jersey.
- A Bachelor's Degree in Nursing or a health care related field is required, although additional related experience may substitute for the degree on a year-for-year basis.
- A minimum of four years of acute care hospital clinical experience is required.
- Recent experience in case management, home health, utilization review, or a unit clinical specialty is required.
- Basic computer skills are required.
- The candidate must demonstrate the ability to work effectively and collaboratively with interdisciplinary teams, along with resourcefulness, effective written and oral communication, negotiating, and analytical skills.
- The candidate must be available to rotate on weekends as part of a team.
- Basic Life Support certification, including CPR, is required.
Responsibilities
- Coordinate each patient's progress through the health care system in collaboration with physicians and clinical staff.
- Ensure resource utilization is appropriate to the patient's clinical and psychosocial needs and remains within covered benefits.
- Develop and implement comprehensive, multidisciplinary plans of care for patients with complex medical and socioeconomic problems.
- Provide real-time interventions to ensure compliance with health care regulations.
- Meet newly admitted patients to identify continuum-of-care and discharge needs, and document the initial discharge planning assessment within 72 hours.
- Identify and document the proper level of care, discuss cases with attending physicians and the physician advisor, and identify anticipated length of stay.
- Work with attending physicians on clinical pathways or MCG guidelines and proactively resolve barriers to care.
- Develop and implement case management plans and interventions and perform discharge planning according to established guidelines, protocols, and timeframes.
- Initiate discharge plans, document interventions, update care plans weekly, and confirm patient face sheet information.
- Assure proper handoffs for transfers and communicate discharge plans to new case managers.
- Assess the need for referrals to outside resources and obtain authorizations for home care, home intravenous infusion, and durable medical equipment.
- Coordinate forms or electronic referrals, arrange transportation, and confirm services and contact information.
- Identify readmissions within 30 days, conduct walking rounds, and identify high-risk patients.
- Work with Social Work and utilization management to reduce length of stay and prevent denials.
- Coordinate and manage distribution of the Important Message from Medicare and assist with the Medicare Outpatient Observation Notice.
- Conduct or co-lead care coordination rounds, promote working relationships with patients and families, and act as a patient advocate.
- Serve as a resource regarding patient classification and reimbursement.
- Use computer programs competently, achieve department dashboard scorecard goals, and escalate avoidable days.
- Complete mandatory trainings and in-services and demonstrate knowledge of safety, security, and infection-control risks.
- Participate in quality improvement by collecting and analyzing data, identifying trends, suggesting improvements, and serving on clinical process improvement teams.
- Maintain compliance with University Hospital standards and stay current on applicable federal, state, and hospital regulations, laws, and policies.
- Perform other related duties as assigned.
Desired Qualifications
- A Master's degree in Nursing or a health care related field is preferred.
- Case management certification from the Commission for Case Manager Certification, the American Case Management Association, or Registered Nurse Board Certified is preferred.
