Requirements
  • Graduate from an accredited four-year college with a major in social work, psychology, sociology, or a closely related field, or graduate from an accredited four-year college with a major in any field and satisfy an alternate qualifying experience or coursework route.
  • Have one year of full-time experience as a social worker, case manager, or care coordinator in a public or private social service agency, or one year of experience in education or treatment of persons served with developmental disabilities or related conditions, or complete at least one course focused on developmental disabilities and provide an official college or university transcript verifying completion.
  • Have a valid driver’s license and an acceptable driving record as determined by Pinnacle Services policy.
  • Have a vehicle covered by current insurance.
  • Successfully clear applicable local and/or stated background checks.
Responsibilities
  • Assess areas of an individual's life where assistance is needed.
  • Offer and coordinate resources for persons served.
  • Monitor service delivery and work with providers to ensure the needs of persons served are met.
  • Develop service plans.
  • Attend regular in-person meetings with persons served and their interdisciplinary teams.
  • Document work related to individuals served in case notes to create billable units.
Desired Qualifications
  • Have one year of experience in education or treatment of persons served with developmental disabilities or related conditions, or complete at least one course specifically focused on developmental disabilities.
  • Possess a Licensed Social Worker (LSW) or higher form of Board of Social Work licensure.
