Requirements
  • A Bachelor's degree in Human Services, Healthcare, or a related field and at least six months of relevant experience; alternatively, a High School Diploma or its educational equivalent and two years of relevant experience including or supplemented by six months of supervisory or lead experience; alternatively, at least four years of experience in a direct care role in a YAI program in good standing.
  • Required foundational certifications include Strategies for Crisis Intervention and Prevention – Revised, cardiopulmonary resuscitation, and YAI-issued Approved Medication Administration Personnel certification for residential assignments or Day Services Model Mentorship Training for day assignments.
  • Newly hired employees must obtain and maintain required YAI certifications within six months of appointment; applicable certifications must be obtained within three months for employees changing programs or settings.
  • Employees must maintain foundational and program-specific certifications.
  • A satisfactory equivalent combination of education, experience, and/or training may be accepted; newly hired employees must be at least 18 years old and obtain required certifications.
  • Excellent written and oral communication skills, including service documentation, progress summaries, and communication with external providers or circles of support.
  • The ability and willingness to lead, direct, and teach others technical skills and knowledge.
  • Proficiency with computers and Microsoft Office Suite, with the ability to learn Workday and electronic medical record and documentation systems.
  • The ability to assist with supports and activities of daily living, including handling bodily fluids in accordance with documented plans.
  • The ability to manage multiple tasks and priorities while addressing changing needs.
  • The ability to work beyond scheduled hours and report on holidays, during inclement weather, or during emergencies to maintain coverage.
  • The ability to remain calm, act assertively, and exercise judgment in stressful situations involving challenging individuals or behaviors.
  • For transportation assignments, a valid driver's license and Van certification must be held or obtained and maintained.
  • The ability to meet the essential physical demands, including lifting or moving individuals, pushing wheelchairs, lifting up to 25 pounds occasionally, and implementing emergency interventions as required.
Responsibilities
  • Perform Direct Support Professional duties to provide supervision, protection, and care for supported people while facilitating activities and community connections.
  • Provide supervision, lead during crises, de-escalate situations, demonstrate applicable competencies, and direct Direct Support Professionals.
  • Implement individualized plans, protective oversight, Behavior Support Plans, Staff Action Plans, and Life Plans, and train direct care staff on interventions and goals.
  • Assist with staffing and coverage, including entering and modifying schedules and securing coverage for absences, time off, community activities, and trainings.
  • Assist with program financial systems, including ledgers, expenditures, petty cash reconciliation, receipts, and electronic transmittals.
  • Coordinate supply needs, including ordering, documenting, and storing food, household, toiletry, and over-the-counter medication supplies.
  • Report facility maintenance, upkeep, and safety concerns and assist with coordinating resolutions.
  • Support facility-related checks by reviewing records and obtaining required documentation.
  • Assist with documenting required staff training, education, and credentials.
  • Provide shift leadership in the absence of the Program Supervisor by assigning duties, assessing work performance, identifying issues, making decisions, and escalating concerns.
  • Complete and maintain required trainings and certifications.
  • Participate in committees, liaison activities, special projects, and agency-wide initiatives when assigned.
  • Facilitate formal agency trainings under supervision when assigned as a certified trainer.
  • Operate a YAI motor vehicle to transport supported individuals when required.
  • Comply with applicable regulatory requirements, including the Health Insurance Portability and Accountability Act.
  • Perform other assigned duties.
Desired Qualifications
  • A Bachelor's degree in a related field.
  • A valid New York State Driver's License is highly preferred for assignments in areas with limited public transportation.
  • Lead experience as a Direct Support Professional and/or supporting people with intellectual/developmental disabilities or supervisory experience in a related area.
