Requirements
  • Some credits from an accredited college or university, or at least two years of experience working with homeless individuals.
  • Ability to work with clients from diverse cultural, ethnic, and socioeconomic backgrounds.
  • Ability to work with homeless clients in the field and communicate effectively, both verbally and in writing, with collaborative entities.
  • Working knowledge of Microsoft Word, Excel, and the Internet.
  • Experience working with homeless persons with co-occurring mental health, physical health, developmental disability, and substance use issues.
  • Ability to report legal issues in accordance with applicable law, including Tarasoff matters, child abuse, vulnerable adult abuse, danger to self or others, and grave disability.
  • Professional experience working within a multidisciplinary team.
  • A minimum of three years of being drug- and alcohol-free is required if in recovery.
  • Ability to produce verification of employment eligibility and pass a background clearance.
  • A tuberculosis test is required within three months before or seven days after the date of hire, with annual renewal thereafter.
  • CPR and First Aid certification are required within 30 days of employment.
  • A valid driver's license and auto insurance are required.
  • Knowledge of evidence-based practices and ability to use them in the field.
  • Ability to work evenings, overtime, weekends, or holidays.
  • Ability to work in homeless encampments and other challenging field environments, including exposure to severe disabilities, infectious diseases, aggressive clients, high-crime areas, moving vehicles, and weather conditions.
  • Ability to carry up to 25 pounds and perform sitting, standing, typing, writing, walking, driving, listening, and speaking tasks.
  • Ability to perform effectively in unpredictable situations, crises, constant distractions, interruptions, and changing priorities.
Responsibilities
  • Conduct immediate-needs assessments with adult consumers and gather information for comprehensive treatment plans.
  • Present cases within a multidisciplinary team and collaborate with internal and external entities.
  • Establish client treatment plans that align with clinical assessments from therapists, nurses, and substance use counselors.
  • Assist consumers in resolving or coping with mental, emotional, medical, and substance-related problems.
  • Document services, treatment plans, notes, and outcomes in accordance with E6 program standards and enter them into the Homeless Management Information System.
  • Provide field-based case management to homeless consumers, including immediate-needs solutions and linkages to benefits, housing, health, substance use, food, hygiene, employment, and school resources.
  • Coordinate daily field work, client services, documentation, meetings, and trainings.
  • Coordinate treatment with the multidisciplinary team as part of the homeless outreach effort.
  • Participate in required program, division, multidisciplinary team, and offsite meetings and trainings.
  • Coordinate transportation and transport E6 staff and participants using the agency van as needed.
  • Maintain documentation of homeless status and provide it to intake workers and clients.
  • Provide HIV/AIDS information, education, and linkages to testing, housing, and services.
  • Enter activity into the Homeless Management Information System regularly.
  • Assist with collecting and entering information for Homeless Services and Housing reports, including Quarterly Performance Reports and Annual Performance Reports.
  • Build rapport with community organizations serving homeless individuals, families, low-income households, and other target populations.
  • Inform, educate, and refer homeless individuals and families regarding HOPICS programs and available services.
  • Maintain client files and records in compliance with HIPAA, 42 CFR Part 2, and funding requirements.
  • Represent the agency and division professionally at meetings and community events.
  • Participate in weekly individual and group supervision.
  • Coordinate submission or transfer plans for client documentation when unable to continue working.
  • Perform other duties assigned by the supervisor or division director and support administrative and data-related Information Technology functions.
