Requirements
  • At least one year of registered nurse experience in acute care hospital, oncology, long-term care, hospice, or home health; school internships and clinical hours may be considered in individual cases.
  • A current license to practice nursing in the state of employment is required.
  • Current CPR certification is required.
  • Current automobile insurance and a valid driver’s license are required.
  • Knowledge of terminally ill patients and their families, hospice services, the roles of hospice disciplines, community resources, general nursing practice, managed care principles, and regulatory guidelines including Medicare, Medicaid, and Accreditation Commission for Health Care standards is required.
  • The ability to perform patient assessments, operate nursing assessment tools and office equipment, document clinical information, communicate effectively, support patients and families during crisis, and maintain transportation for work is required.
  • The employee must complete a Florida Level 2 background screening through the Care Provider Background Screening Clearinghouse.
Responsibilities
  • Perform comprehensive patient assessments and reassessments, including in-person and telephonic registered nurse assessments and screenings, and educate patients, family members, and caregivers.
  • Deliver skilled nursing interventions and symptom management aligned with goals of care.
  • Develop and update individualized plans of care, including patient visit frequency and risk identification.
  • Coordinate care across interdisciplinary teams, including consultation with physicians and nurse practitioners.
  • Provide patient and caregiver education regarding disease processes, self-care techniques, end-of-life care, medications, nutrition, and dietary needs.
  • Communicate changes in condition and signs or symptoms of decline to the appropriate provider, such as a nurse practitioner, primary care provider, or hospice physician.
  • Support continuity of care by recognizing and supporting home health, palliative, and hospice referrals and transitions between levels of care.
  • Document timely clinical records, including screening and assessment findings, physician orders, progress notes, and care plans.
  • Maintain effective working relationships with the interdisciplinary group and involve the team when the patient’s care plan requires team participation.
  • Participate in on-call and patient support activities and conduct on-call services in a clinically competent and responsive manner.
  • Deliver and document hands-on nursing care according to individualized plans of care in patients’ homes, assisted living facilities, or other patient residences.
  • Provide emotional, physical, and spiritual support, education, comfort, and bereavement guidance to patients and their loved ones.
  • Teach caregivers how to provide safe and confident care for their loved ones.
  • Maintain accurate clinical documentation and follow infection-control, compliance, quality-improvement, and scheduling standards.
