Requirements
  • A Doctor of Pharmacy degree is required.
  • Completion of a PGY2 Pharmacy Specialty Residency, or completion of a PGY1 Pharmacy Residency plus two years of relevant experience, or completion of no residency plus five years of relevant experience is required.
  • A pharmacist license in the State of Pennsylvania is required.
  • Knowledge of and ability to apply pharmacokinetic principles for monitoring and dosing medications is required.
  • Ability to monitor patient response to medications and provide recommendations on management when appropriate is required.
  • Ability to identify and document sentinel or serious events, near misses, medication errors, and adverse drug reactions in the electronic event reporting system is required.
  • Ability to provide quality patient care in accordance with standards of practice, demonstrating clinical expertise, competency, knowledge, and skills is required.
  • Ability to promote and assure compliance with the hospital formulary and policies and procedures defined by the Pharmacy and Therapeutics Committee is required.
  • Ability to provide evidence-based recommendations to support patient care is required.
Responsibilities
  • Oversee the medication distribution system for the assigned area of responsibility.
  • Identify, prevent, and resolve drug therapy problems by ensuring each medication is indicated, effective, safe, and convenient for the patient.
  • Interact with co-workers, visitors, and other staff consistent with the values of Jefferson.
  • Assure the appropriateness of drug therapy and compliance with hospital policies.
  • Oversee the distribution of medications throughout Thomas Jefferson University Hospital.
  • Support implementation of processes that assure the quality of pharmacy services.
  • Train pharmacy technicians, residents, students, interns, and new pharmacists as assigned.
  • Precept student pharmacists and residents as assigned.
